Why there's no upload button here: an IEP is a protected education record. Rather than route your child's file through a third-party form service, email it straight to me at schoolaid@learningztuff.com — that keeps it in one place I control. I'll show you that address again on the next screen. It's used only to build this student's plan, never shared with anyone, and deleted the moment you ask.

These are the federal IDEA eligibility categories. Pick the closest one, or leave it alone if you're not sure — the next question matters more.

The everyday version. "Melts down when writing is involved." "Can read the words but can't tell me what happened." That's more useful to me than a label.

These two numbers are usually very different, and the gap tells me how to pace the week.

What to avoid, and what helps

Four questions, all taps. This is the section that keeps a lesson from going sideways on a Tuesday night — the wrong task in the wrong shape can end a session before it starts. If none of the options fit, say so and move on.

Movement & regulation

My background is in exercise science as well as inclusion — movement breaks are built into every plan unless you'd rather they weren't.

A goal tells me more than a subject does. "Gets through homework without a fight." "Reads a chapter and can actually tell me what happened." "Focuses better in school." If it really is about one subject, say that instead — either works.

Interests & obsessions

This is the most important section on the form. Everything here becomes the theme of the lessons — the word problems, the reading passages, the characters, the rewards. A kid who won't do a math worksheet will absolutely do a Minecraft math worksheet. Be as specific as you possibly can.

What are they into? *

The one thing they'd bring up unprompted at dinner. That's the highest-leverage detail on this whole form.
